New Student - Identification Required
Name and Date of Birth
Please submit photocopied evidence of your full name (including any changes to your name) and date of birth. This must be one of the following:
- Birth Certificate
- Passport
- Driving Licence
If you have changed your name, please supply a copy of one of the following to support your change of name:
- Marriage Certificate
- Deed Poll
- Statutory Declaration
- Affidavit
